The Foundation recognizes that the health of an individual or community is about much more than what happens within the walls of a hospital. Our team strives to look at the entire environment and health system and all of the components that help support individuals in their health journey.
In 2020 the Foundation recognized that patients were falling through the cracks due to a lack in services for basic care coordination. This began the development of what is now called Connected Care. Connected Care is a team of professionals dedicated to walking side by side with patients to make referrals, develop individualized care plans and assist with every day obstacles of accessing services.
